Identifying and leveraging community and organizational strengths to advance health and health care.
Our mission is to support the sustainment and prosperity of rural communities through the improvement of individual, population, and community health.
Our approach is to facilitate systems-based improvement initiatives that are inclusive of all stakeholders to address interrelated economic, health, education, and cultural factors that shape the quality of life for individuals and families. Engagement of patients and families in the co-design and co-production of health and healthcare is a priority. We recognize the importance of addressing social determinants of health and their link to the efficacy and safety of health care.
